This 57 year-old Marin County woman had two previous breast implant operations and presented a most challenging revision. The initial breast augmentation was done in San Francisco in 1981 with silicone implants, which were switched to saline implants in 1996 due to capsular contracture. She was seen in our Marin County Plastic Surgery office with a deflated left breast implant and a contracted right breast implant. At surgery, after removing the implants the skin envelopes were markedly deflated and different. The right breast hung much lower. New, textured, highly cohesive implants were placed and an anchor mastopexy was done to improve shape and symmetry. The patient is thrilled with the new shape and softness of her breasts. She is shown 2 months after surgery.
